OK, just to clarify a bit.. Most 5-man instances are run on Group Loot, which means that when a boss drops a green/blue item, all party members will get the Need/Greed box. They can use this dialog box to roll on the item. If all players hit Greed, the high roller wins it. If 3 players hit Greed and 2 players hit Need, only the last 2 get to roll - again with the high roller winning the item.
Note that the game rolls the dice for you, all you have to do is indicate Need or Greed. And no, there is no reason to suspect that the last person to Need will get the item - it's all random.
Also note that many groups are run with the policy that everybody Passes on Bind on Pick-up items, which most blue boss drops are - and then use Need/Green for everything else, including random greens that drop from trash mobs. Passing on BoP-items is imo a much safer system, as it allows for time to discuss who should be allowed to roll on the item. With BoP items, the item transfer is permanent, so I avoid it at all costs on boss drops.

This seems to be more common on older servers, I haven't been in a group like this for quite some time. Honestly though I prefer to have everyone roll need or greed at the start rather than passing. Sure you run the risk of a hunter winning your rogue dagger but by passing you run the risk of losing an item to a ninja that waits until everyone else passes then hits need. At least the hunter in the first situation will use the item.

If everyone pass the items, doesn't that make the loot ''free for all'' therefore allowing anyone to just go near the corpse and shift click and ninja the whole thing?
I don't really see how this is a ''Safer'' system to avoid ninja.
I'm on a old server and I've never been in a group where we were passing on all bop items. We use the DESIGNED system as if we need, we hit NEED, and if we don't need it, we either hit Greed or ''x'' to pass.
I can only think of 1 single time that a rogue rolled Need on the arugal cloth robe in SFK AFTER saying he was going to pass and stole it from the mage that needed it.
What did I do? /ignore that stupid rogue and I took my lvl 60 and run it again for the mage and he got it.
It's not the end of the world if something like this happens. EVEN if it's a OMG freaking epixx purple 1337 item that's going to change the whole world.
Unless it's the sword of the thousand truth, it won't make you invinsible. It's just a damn item that is going to become obsolete one day.
If I would be in a group that people need to pass on everything because they're too retarded to figure if they need or can use an item, I'd just leave.
When I go into an instance, I tend to try to waste the less time possible on looting, and discussing about each BOP that drops to say who will benefit the more out of the item or who this item will be a ''bigger'' upgrade for. If it's an upgrade, I just click need and wish that I win the roll.
I don't really care if the other player wear a green robe and I already wear a blue one. I worked the same he did and I'm entitled to get upgrades also.

Your line of reasoning is absurd.
Every single Tier 3 item in Naxx will become obsolete in a few months. If someone ninjad one of the tokens from one of our Naxx runs, they'd be driven off the server.
Low level instance loot may have little to no value to someone who has played the game through with one, two, or six toons. But for someone starting out, or someone playing casually (for whom that loot may last a month), it can be a pretty big deal.

I should add that the "passing on all BoP-items" system is by no means safe from ninjas, and it should only be applied if you have a reasonable degree of trust in your teammates. Also, the act of ninja'ing is a matter of principle, and it doesn't really matter if it is a crappy Deadmines green or a token from Naxx. I don't subscibe to the notion of "if you can't beat them, join them", so I'd rather pass on a BoP like we've agreed to and let somebody else possibly ninja the item (and get kicked) than I would risk being considered a ninja myself.
But hey, that's just me. And if you run instances mostly with your guild, 99% of these ninja concerns disappear.

I personally new someone on my old server who just decided one day to become a ninja. He went into groups ninja'd every time he had an opportunity. He waited until his reputation got so bad that he couldnt get any other PUGs then used the paid character transfer service went to another server and changed his name. His reasoning was simply "I am farming shards."
You could imagine how much people hated this individual, but they were capitalizing on the old server mentaility of always passing on BoP's.

Learn them and live by them. That does not mean the master looter is perfect or anything of the sort. I have seen the master looter lock up on the final boss in scholo and loose all of the loot ( none of which he could use, and blizzard does not fix these things ). I have seen a carapice given to the wrong person because the master looter missed a roll. I have personally given a blue bop to the person adjacent to the desired target the first time I was ML on the Baron run.:embarassed:
Master Looter is not perfect, but it does cut down on ninja problems (only potential problem is a ninja ML) and can speed things up if you have an experienced ML. First time ML can be a problem, as I can attest from my first experience.
A hint, ML should be someone who does not have mana for optimal efficiency. That is not always possible, but it does speed things up.
I also like groups with a core of people from the same guild and one of them is ML. The biggest weakness of Master Looter is for the ML to be a ninja, but I have not experienced that disaster.
