I'm new to the game, how do you find out what server you are on?

Or is the server called 'Realm'?

Yes, Server and Realm are the one and same. Just a more fantasy-ish name instead of the more technological "server" we use in the modern world. :P

On the log in screen you can see your current servername in the lower right.
