I've just entered the Outlands (59 Blood Elf Hunter) and have come into contact with the dreaded "reputation"!

I remember reading somewhere (that I cannot now find because my search-fu is weak) that reputation should be obtained in a particular order (do grinding before instances or similar) but I can remember neither the specifics nor what reputations that advice applied to. Is anyone willing to explain the best way to go about improving faction in the Outlands? Also, should I really be bothered about reputation? Or should I just let it happen as it progresses?

If this makes no sense at all then I apologise ... really I have no idea what I'm doing so my terms of reference are not likely to be very exact.

dont worry about reputation. All requirements were lowered, so what you pick up automaticaly trough questing and the occasional instance will be more than enugh.

if you want some of the rewards you get at exalted, you may want to look into it. Then, the normal order would be to do instances of that region till you hit 5999/6000 and then start questing.

No need to worry about reputation. Just do all quest that you come across. The quests will give you the reputation you need. From time to time run instances occasionally. Currently you are probably in Hellfire Peninsula. Just continue doing all the quests there and run instances from time to time.

I went in order like so:
Hellfire Peninsula-->Zangarmarsh-->Terokkar Forest-->Nagrand-->(Your Choice)

Reputation That You Need:
Thrallmar
Cenarion Expedition
Sporeggar(Not Required. I am Unfriendly)
Keepers of Time
Lower City
The Sha'tar

This is just a simple overlook of what I have. All you have to do is get honored with each.

So let's back up a second. There are only really 2 reasons to get rep with most factions: heroic keys and rewards. AS twoflower noted, the quests in a zone will get you to honored with that faction - Hellfire quests with Thrallmar, Zanger with CE, etc. So that takes care of honored and the heroic keys, esp since you're going to hit instances now and then for fun and drops.

The reason to go beyond honored is for the rewards. There are three kinds of rewards: Patterns, items and enchants.

Patterns: Take a look at your professions (or those you plan to switch to at 70 if you're doing that) and see who has patterns you want. Grind rep with those factions.

Items: Same for items, but keep in mind that there might be other comparable items from 70 instances, etc. For example, I ground Aldor rep for the exalted sword. But with the Season 1 Arena swords now available for honor from the BGs I wouldn't do that - I'd grind in the BGs since the S1 swords are better than the Aldor sword. Similarly, there's a Lower City healing weapon at Exalted LC. But there's a very good S1 version too. For pretty much any weapon, the S1 stuff from honor is an alternative you should consider.

Enchants. Finally, there are some enchants that have nothing comparable. Aldor and Scryer give enchants for shoulder slots that have nothing comparable from elsewhere. CE rep gives a great head enchant for rogues and others who want hit and AP.. there are other head slot enchants from other factions.

i'd say only worry about the enchants and maybe in second priority about the patterns.

There are better items out there, as clevins pointed out. and that is for ALL items you can get via reputation. Patterns are not that important either. if you cannot create a item, someone else on your server can.

But with the head and shoulder enchants, you will be stuck with them for the rest of the time that you play BC. So choose around these as top priority.
